Please let us know if you change your name, address or telephone

Please do everything you can to keep appointments. Tell us as soon as possible if you cannot, otherwise other patients may have to wait longer. |

Remember appointments are available to individuals. To avoid delaying other patients' appointments, you may be asked to make a separate appointment if you have a number of problems or require special services. |

For x-rays and tests, you will be asked to telephone for results. Please do not phone for results before this time as test results take time to reach us. Enquiries about tests ordered by the hospital should be directed to the hospital, not the practice. |

If you are referred to a specialist in the hospital, please understand we have no control over the speed with which you will receive an appointment. This is a constant cause for concern for family doctors and we will occasionally need to refer you through the casualty department in that particular hospital. You may be seen by a specialist and are put on a waiting list for a procedure. If the delay is concerning you, you will need to discuss it with the hospital as the control is out of our hands.
